Job Description

Meet regularly and as needed with the Clinical Team Managers (CTM)

Identify weaknesses and provide guidance in developing CTMs proficiency

Analyze Care Management quality monitoring reports regularly

Provide feedback on compliance and develop improvement strategies

Participate in monthly and as needed auditing reviews of members' records

Assure timely identification of problems and correction of deficiencies

Monitor unusual occurrences and report follow-up procedures

Participate in special projects as assigned by Sr. Dir of LTSS and Clinical Operations Leadership

Participate in quality initiatives and activities as designated by Quality Improvement Subcommittee

Participates in hiring process


RN/BSN/BA in Nursing and/or Business or Healthcare Management

Active and unrestricted NYS RN license

In depth knowledge of the health care delivery system

Knowledgeable of Long-Term Care services and regulations

Familiar with current community health practices for the frail adult population, behavioral health and cognitively impaired seniors

Adept at operating within a diverse and multi-cultural work environment and community of members such as Spanish, Chinese, Russian, Creole and Korean

Strong written and verbal communication skills

Proficient in the use of Microsoft office Suite tools
